This only affects 50-pack boxes with lot number 22219P shipped between 6/20/2020 and 7/17/2020. You can identify the lot number on the outside of the 50 pack carton and/or on the back of each sachet, near the ‘best by’ date.
Please email a phone shot of the lot number on your Elovate 15 and a copy of your transaction to email@example.com with the word ‘Replacement’ in the subject line. Also, include your name, email address, and shipping address to ensure your replacement is handled quickly. We request that you simply destroy and discard the effected product promptly and do not attempt to use it.
